Health Promotion Program
The Health Promotion Program (HPP) at Fred Victor collaborates with both Fred Victor programs and community stakeholders to enhance health and well-being across the GTA. The program plays a vital role in every stage of program development, from conducting needs assessments and developing curricula to implementing and evaluating initiatives. HPP works closely with diverse groups to address systemic gaps in services through policy consultation, advocacy, and capacity-building efforts.
In addition to these core activities, the Health Promotion Program is actively involved in civic engagement, group facilitation and training, community development, and project coordination. By fostering partnerships and building connections, HPP helps strengthen the community’s ability to respond to health-related challenges effectively.

Improving Health Access (Geriatric Care)
Our Psychogeriatric Case Workers provide essential support to senior patients transitioning from hospital to home, ensuring a smooth reintegration into their community. In addition to assisting recently discharged patients, they also offer ongoing support to seniors in the community who are not currently connected to care.
By providing personalized care and connecting seniors to necessary health and social services, our case workers help enhance the well-being and stability of vulnerable older adults.
