Downtown East HART Hub
What is the HART Hub?
The HART Hub provides integrated health and social supports for people experiencing homelessness, mental health challenges, substance use, and other complex needs in Downtown East Toronto.
Co-led by Regent Park CHC and Fred Victor, and supported by the Downtown East Toronto OHT, the Hub connects people with outreach workers, case managers, and other health care providers.
Who We Serve
- People aged 16 and older experiencing complex substance use and/or mental health challenges
- People living in, or connected to, Downtown East Toronto
- People ready to engage in treatment and recovery

- Case management and counselling services across the continuum of care
- Access to primary care
- Group programming
- Street and community outreach
- System navigation
- Connections to long-term housing and community supports
- Crisis management
- Peer support
